Enrollment Instructions for Residents & Non-Residents for Edmentum Virtual Option 2020-21

    For all students wanting to enroll in Edmentum Virtual Option for 2020-21, you will need to submit the following information pending your residency and enrollment status.

    • BVA Online Learning Application (Page 1)

    • Online Learning Policy Agreement (Page 2)

    • Technology Lending Form (If you need to borrow a chrome book)

    • A printed copy of Pre-Enrollment Report – (Signature required)

    • Birth Certificate

    • Driver’s License – To verify the relationship (identify parent on the birth certificate)

    • Request for Records Form

    • Proof of Residency– Provide 3of the following in the PARENT or GUARDIAN’s name:

      • (1) Primary Residency Document: (Options noted below – ONE)

        • Property Tax Statement or Warranty Deed

        • Lease – Must have signatures, valid start and end dates of lease & all occupants’ names.

        • If you RECENTLY closed on a home, a copy of the Property Tax Transfer Affidavit or Warranty Deed could be accepted from your closing documents. We cannot accept a Purchase Agreement. (Within 60 days from Closing).

      • (2) Additional Supporting Documents: (Options noted below – TWOare required.)

        • Utility Bill – FULL Statement (Gas, Electric, Water or Cable/Internet) – If you pay your utilities electronically, you can find a current monthly statement in your account history to print/forward. Please include the page with the service address noted.

        • If you have recently closed on a home (within 60 days); Utility activation confirmations will suffice for gas, electric, cable/internet or water.

      • Physical Examination – Kindergarten only

      • Vision Screening/Test – Kindergarten only

      • Prior Care Form – Kindergarten only

      • Immunization Record – Must be current and up to date for all new enrollments.

      • Recent Report Card for Grades 5-8 Only

      • Transcript and Discipline Report for all incoming students entering Grades 9-12

      • Most Recent IEP – If your child receives special services, a copy of a current IEP is required at the time of enrollment.

      • MHSAA Transfer Form – for all students entering Grades 9-12

    Once your application and information has been reviewed – you will receive notification via email or US Mail. If approved, the notification will include instructions on how to proceed with the enrollment for your student.
