Plan ahead for next school year when visiting the doctor's office for sports physicals and well checks this summer! Take a copy of the BAS Medication Authorization Form with you for any prescription or over the counter medications your student may need during the 23/24 school year. For other student medical needs during the school day, physicals, or emergency action plans, we have all district information and related forms posted here.Attention all incoming 7th grade families:All 7th graders must have updated vaccine records to comply with The State of Michigan public school enrollment guidelines. Records must be up to date to avoid potential exclusion from school. Please choose the option best for your student(s) prior to the start of next school year:1. Schedule a healthcare provider visit to update required vaccines2. Make an appointment with local health department to update required vaccinations or obtain an updated vaccination waiver (existing waivers must be updated to comply with 7th grade vaccine requirements)See the flyer below for more information on vaccine requirements and how to schedule an appointment with The Livingston County Health Department.

All student medications that may be required while in school must be provided by a parent/guardian and have an accompanying medication authorization form signed by both a parent/guardian and a health clinician.
